HIV+ Case Management

Our Medical Case Management Team provides the necessary advocacy, support, and services tailored to the needs of people living with HIV/AIDS. By delivering care personalized to each client's individual needs, our case managers achieve the highest levels of care coupled with compassion.

What Do Our Medical Case Managers Do?

Our case managers complete a myriad of services centered around the advocacy and care for our clients, to learn more about any one of the services below, contact us!

  • Help coordinate service including appointments with an Infectious Disease Doctor to set up access to labs and medications.

  • Developing treatment plans that address and assist in overall treatment adherence.

  • Navigate health insurance systems, MaineCare, and other social support services.

  • Help patients access community resources including primary care providers, mental health resources, and substance use support.

  • Set up state-funded transportation to medical appointments.

  • Food pantry and nutrition services that adhere to the specific nutritional needs of people living with HIV/AIDS.

  • Signing clients up to receive assistance from the Ryan White Program, including ADAP, housing, dental, and food assistance.

Ryan White HIV/AIDS Program

The Ryan White HIV/AIDS Program is a nationwide program named after Ryan White, an American teenager that was diagnosed with AIDS after receiving a contaminated blood treatment for his hemophilia in 1984.

The program began to provide assistance to people living with HIV/AIDS in 1990 with the passing of the Ryan White CARE Act.

Our service area includes Washington, Hancock, Penobscot, Piscataquis, and Aroostook Counties.
